This Cara Bramlett case study is about one of my own students, and her story is proof that a specific, underserved niche can turn into a six-figure author business.
Cara is a physician assistant and clinical program director who followed my teachings and became a six-figure bestselling author. She found a gap that, looking back, seems obvious: there was no leadership book written specifically for medical professionals. She wrote it, and it took off, including bulk orders from hospitals and universities, and being cited in doctoral dissertations.
I had her on my Author Success Stories series to find out how she did it. The full interview is right here, and below it I've pulled out what I think any nonfiction author can copy.
One note before we start: this interview comes from Bestseller Empire, the pre-AI version of my course. Golden Bestseller Empire is its next chapter. The methods are exactly the same, and they're timeless: finding the gap, researching it properly, building a launch team and giving readers a reason to join your list. What's new is that you now have AI-assisted tools and 31+ one-click generators that make every one of those steps even easier.
Who is Cara Bramlett, and why is her story worth studying?
Cara Holcomb Bramlett is a physician assistant by trade, still practicing medicine one day a week, while spending the rest of her time as a clinical program director working in population health. She's also pursuing her doctorate and is about to start her dissertation.
She has written two very different book series. Her leadership series includes Leading with the Clinical Mind (her first book) and her breakout bestseller, Servant Leadership Roadmap: Master the 12 Core Competencies of Management Success with Leadership Qualities and Interpersonal Skills, plus a second book on mental toughness. Separately, she started a second series, Emotional Trauma No More, about recovering from narcissistic abuse and complex PTSD (C-PTSD), drawn from both her clinical background and her own personal story.
You can see her full catalog on her Amazon author page.
Here's the part I love most: Cara did not start as a known author. She started with a felt gap in her own field, my course, and the willingness to write the book she wished had existed when she needed it.

How did Cara learn to do all this?
She learned it from me. Cara went through my course and was active for years in my Facebook group, long before we recorded this interview together. She posted this update to the group when her royalties crossed six figures:
"It's official, I'm a 6-figure best-selling author! I'm often asked, is it worth it? Does Gundi Gabrielle's program work? YES! YES! YES! This is LIFE CHANGING MONEY! I'm absolutely so blessed and thrilled to share with everyone the impact Gundi's Guiding Best Seller Principles in action. It is such a blessing to share my writings with the world (literally!). Thank you to Gundi and this special group. PS- did I mention this is on AUTOPILOT?!?!"
Her own KDP Royalties Estimator, shown in that post, put her lifetime estimated royalties at $102,205.81 across 5 books (August 2017 to February 2024). At one point, her Servant Leadership Roadmap sold around 200 books in a single two-day stretch, driven by institutions bulk-buying it for their teams.

Why did Cara find a gap nobody else had filled?
In her own words: when she moved into management, there weren't many clinical professionals like her, nurse practitioners or physician assistants, in leadership roles. She noticed that she and others like her tended to lead the way they'd been trained to treat patients, and that style doesn't transfer to leading employees.
So she set out to make leadership relatable specifically to people in the medical profession. Throughout her book she takes a leadership concept or theory and relates it to a clinical mindset her readers already understand, an approach she calls one of the most powerful tools she's found as a mentor to people new to leadership in healthcare.
There simply wasn't a resource out there for people like her. She provided it.

Why did she choose servant leadership specifically?
Cara explained that servant leadership fits the personalities of people drawn to caregiving professions. Caregivers, she said, tend to naturally have a servant heart, and that style shows up constantly in clinical and medical disciplines.
Within the book, she doesn't stop at servant leadership. She also covers other leadership styles for different situations, for example pulling in a more authoritarian style during a medical code, where there's no time to negotiate which medication to give. Relating those different styles back to real medical scenarios is, in her words, what makes the biggest impact on her readers.
What made the book take off?
The book is relatively short, about 120 pages, written in a conversational style that opens with an example many readers can relate to. That combination, a real gap plus an accessible read, is what drove its growth.
Cara regularly gets emails and LinkedIn messages from people who've adopted the book for a leadership program or initiative at their organization, and order it in bulk for their teams. Surprising even to her, colleges and universities have picked it up and made it part of their curriculum, and more than one person has told her they cited her book in their doctoral dissertation.
The momentum hasn't been a one-time launch spike either. A year and a half after publishing, Cara posted this update:
"I've just sold another 200 books over the past 2 days and today I'm at over $3400!!! This is crazy and on autopilot! The course works! 1 1/2 years later I still have a bestseller and now have 5x my email list! Many authors fail after the initial release and their books are lost. This course is your tool to keep selling long after your release through the easy system Gundi teaches. Bonusβdiscover how to build your personal brand, even if you don't have one now! Thanks Gundi for everything!"

How did she approach the cover and title?
Cara worked with a designer through 99designs for her cover. For the title, she researched what the leadership space was talking about at the time she wrote it: not just "servant leadership" broadly, but specifically leadership competencies, which was a major theme in HR and corporate leadership content in 2018. That's why the title centers on competencies rather than just the servant leadership label.
For the cover colors, she chose black and red instead of her favorite color, pink. Black stands out against Amazon's white background, and red, in her words, is "the leadership color." Simple, clear and easy to read, even from a distance.

How is she using her email list to plan her next book?
Inside the book, Cara links to her email list through AWeber, and she sends personalized, conversational emails to her subscribers. One of the questions she regularly asks her list: "Tell me what you struggle with in leadership. Tell me what keeps you up at night."
The responses pointed again and again to the same theme: controlling emotions. That reader feedback is now shaping her next book in the series, on emotional control in leadership, drawing on her ongoing doctoral work along with her clinical and leadership experience.
This is exactly the lesson I teach in my course: put a lead magnet inside your book, build your list, then ask your readers directly what they need next. They'll tell you what to write.
What's next for Cara beyond the book?
In our interview, I pointed out how much further this could go. Cara already has the authority that comes with a published, cited, curriculum-adopted book, which opens the door to executive coaching, consulting, and paid speaking, all high-ticket services that build on the book rather than replace it.
There's also room to grow the book's reach itself: reaching out directly to hospitals, healthcare systems and universities who could adopt it for their programs, or connecting with thought leaders in the medical leadership space who could introduce it to their own audience. Cara is also exploring BookTok on TikTok as a newer channel, something she pointed out isn't just for younger readers.
Why did she start a second, very different book series?
Cara's second series, Emotional Trauma No More, is about healing from narcissistic abuse and complex PTSD (C-PTSD), the chronic, long-term trauma that comes from sustained mistreatment, as distinct from the single-incident trauma more commonly associated with PTSD.
This series comes from both her clinical background and her own personal story. As she connected with other people who'd lived through the same experience, she noticed how many high-performing professional women shared her story, and she wrote the book she wished she'd had earlier in her own journey: what you'll experience, why it's happening, what's happening in your brain and body, so readers can understand their own symptoms sooner.
One reader's note, which Cara shared in the book itself, captures the impact:
She couldn't stop crying through the first chapter. She felt it was her life. She couldn't explain what was happening and why she responded to others. She couldn't understand why she had stayed so long. She shared that, finally, she did not feel alone and like she was losing her mind. She requested the book be added to her church's resource center.
What can you learn from Cara Bramlett's case study?
If you take only a few things from Cara, take these:
- Find the gap inside a big market. Leadership is a crowded topic, but leadership written specifically for medical professionals wasn't.
- Write from real experience. Cara's credibility comes from being a clinical professional herself, not from research alone.
- Use analogies your specific reader already understands. Relating leadership concepts back to medicine made the ideas click for her audience.
- Build your list, then ask your readers what they need. A lead magnet in the book plus one well-placed question surfaced her entire next book topic.
- Target institutions, not just individual readers. Hospitals, universities and organizations buying in bulk and adopting a book into a curriculum can outscale individual sales.
- A second, different topic can still work. Cara's trauma-recovery series stands entirely apart from her leadership books, built on a separate but equally real personal and professional foundation.
Cara's results are her own, and results vary from author to author. See my Earnings Disclaimer.
How does this fit Amazon's new book algorithm?
Cara's books work because they describe clearly who they're for and what specific problem they solve, for medical leaders in one series, for survivors of chronic abuse in the other. That matters even more now, because Amazon is moving toward relationship-based discovery, where a book is understood by who it's for and what it solves, not only by keywords. I've explained how that works in How to Get Your Book Page Ready for AI Shopping.
This is exactly why I built Golden Bestseller Empire the way I did. Your Book Planner blueprint is turned into an algorithm-ready outline and a knowledge graph. That same blueprint then feeds your book description, A+ content, backend keywords, translations and Amazon ads keywords. I don't know of another app that does that.
And your book stays yours. The Writing Studio works like a word processor where you write or dictate, with an AI Writing Coach and an AI Editor on your side.
